"Urinary ketones" refers to a substance called ketones that can be found in your urine. These ketones are produced when your body doesn't have enough carbohydrates to use for energy, so it starts breaking down fat instead. The presence of
urinary ketones can be an indication of certain medical conditions like diabetes or a low-carb diet.

A recent study found that
urinary ketones tend to be highest in the early morning and after dinner on a ketogenic diet (38).
Diet adherence was evaluated through weekly
urinary ketone measurements, dietary compliance checklists, and bimonthly Food Intake Records.
In a one - week study, overweight people with type 2 diabetes who limited carb intake to 21 or fewer grams per day experienced daily
urinary ketone excretion levels that were 27 times higher than their baseline levels (7).
While urinary ketone excretion means that body fat is being excreted causing fat loss, the effect is minimal: Based on recent studies, the number of calories lost in the urine as ketones is not significant (100 kcal at most).
However, this effect is not significant and the energy lost
as urinary ketones is no more than 100 kcal.
BHB measurement is generally more stable and reliable than
urinary ketone measurement because the latter fluctuates more (Urbain and Bertz, 2016).
If you are keto - adapted, you may not be able to measure
any urinary ketones.

no urinary ketones.
This is completely different to
urinary ketones, as they may be higher or lower.
Ketostix is not accurate and you may be in deep ketosis while showing almost
no urinary ketones.
When ketones are made from the breakdown of fat in the liver,
urinary ketones are excreted and may contribute to fat loss.
And through a study consisting of low carb, high protein diet the results garnered showed, that ketone bodies had reduced in the first 3 months and over a period of time
the urinary ketone concentration reduced, ultimately not showing in the urine results.
The diet was ketogenic, confirmed by
urinary ketones... of note, presence of urinary ketones is a better indicator of ketosis than any information about diet (although they were advised to start at < 20g carb / d).
The lady did not show any improvement of symptoms, but researchers also couldn't detect
any urinary ketones, weight loss or changes in liver function, suggesting that somehow she managed to resist going into ketosis.
