enhanced medical home providing integrated
care for families in poverty is informed by the understanding that emotional
care of the family, including recognizing maternal depression, is within the scope of practice for community pediatricians and that the effects of toxic stress on
children can be ameliorated by supportive, secure relational health during
early childhood.

Concrete actions by policy makers, already practised in many counties, are: matching paid parental leave to the rate and duration observed in Scandinavian countries; providing adequate public funding and developing tax policies that allow parents to make appropriate
child - rearing choices, paying greater attention to
children from poor or diverse backgrounds; integrating
child care and
early education under one ministry or agency and thereby
enhancing quality, qualification requirements, accessibility and affordability.
The contributors to this issue of Zero to Three illustrate the diverse settings and situations that
early childhood professionals confront in their work with young
children and families: A busy morning in a pediatric clinic; the traumatic aftermath of domestic violence; guiltridden parents worried about the health of their infant; mothers with severe psychiatric disorders; reflective supervision in an
early intervention program; and peer - group support for
enhancing social and emotional development in
child care settings.

This brief highlights many notable and emerging successes of grantees in expanding and sustaining services for
children and families in the five core Project LAUNCH strategies (screening and assessment;
enhanced home visiting through increased focus on social and emotional well - being; mental health consultation in
early care and education programs; family strengthening and parent skills training; and integration of behavioral health into primary
care settings).
Five key strategies guide communities in creating a vision for
early childhood social - emotional health with a focus on reducing physical and mental health disparities, especially among vulnerable populations: 1)
Enhanced home visiting through a focus on social and emotional well - being, 2) Screening and assessment in a range of
child - serving settings, 3) Integration of behavioral health into primary
care, 4) Mental health consultation in
early care and education settings, and 5) Family strengthening and parent skills training.
Since 2008, the project has funded mental health consultants (MHCs) who provide training, infant and
early childhood mental health consultation, and referrals to
enhance the capacity of mental health providers,
child care providers and home visitors.
The
Early Childhood Mental Health Consultation Project seeks to
enhance the awareness of the mental health needs of young
children and their families, enable stable and continued enrollment of
children with social / emotional challenges in
child care settings from birth to 7 years old, and strengthen the capacity of individual agencies to address the needs of young
children.

Unfortunately, in light of the generally mediocre quality of
care in the U.S. and the strong association between the quality of
care and its cost, it is difficult for families who need the best
care for their
children to find it at a manageable price.9 This is where broader public policy that
enhances investments in
early childhood development can enable such families to find the quality of
care they seek at a cost they can afford.
In addition, as Belsky notes, this association may be further influenced by broader sociocultural values concerning out - of - home
care for very young
children, the participation of women in the workforce, the normativity of dual - career families, and the extent to which
early child care is perceived as custodial or development -
enhancing.
