The Indian Point nuclear power plant has a long history of accidental radioactive
leaks and spills: spent
fuel pools at the plant housing toxic nuclear waste have been
leaking since the 1990s; corroded buried pipes have sprung radioactive
leaks; tanks have spilled hundreds of gallons radioactively contaminated water; and malfunctioning valves and
pumps have
leaked radionuclide - laden water.

pumped at the tank when
fuel gets low to maintain pressure so I presume I have a
leak.
Kinks /
leaks in the
fuel lines may also prevent the

fuel injectors,
fuel leaks in the
fuel lines, faulty
fuel pump check valve or faulty
fuel pump accumulator.

fuel injector malfunctioning
fuel pressure regulator clogged
fuel filter malfunctioning
fuel pump spark plugs, wires, cap, rotor, coil Vacuum
leak dirty throttle body malfunctioning fast idle thermo valve (similar to vacuum
leak) low oil (unless there is an oil level sensor) belts gaskets seals rings low compression bearing damage (rod knock) Broken cam rocker fell / broke off broken valve spring stuck valve critical engine damage (windowed the block) wiring, fuses, relays starting system - ignition switch, starter, relay Cooling system - clogs,
leaks, flow (may throw a code if you overheat) transmission clutch (es) driveshaft, differential, axles brakes suspension tires hvac lights gauges
Most likely as Handy Howie points out, it could be
fuel leaking through the
fuel pump into the crankcase of your engine, raising the apparent amount of oil, but now it would be diluted with
fuel.
These vehicles have a faulty return port on the
fuel pump module may crack, which could cause a
fuel leak.
If it does
leak down then the
fuel pump internal check valve is bad and
fuel pump needs replaced if the pressure now stays up and does not
leak down then the
fuel pressure regulator has a
leaking pressure seat and needs to be replaced.
If the pressure drops to zero when the key is turned off then you have a
leaking fuel pressure regulator or
fuel pump.

leaks, failed
fuel pumps and other maladies that have afflicted their various Volkswagen models during the years.

fuel rail return line will need to be clocked off to isolate the
fuel pump to see if it still
leaks down.
Stalling at idle could be due to a faulty mass airflow sensor, faulty camshaft or crankshaft position sensors, worn spark plugs, wires or defective ignition coil (s), inadequate
fuel delivery or pressure (clogged
fuel filter, injectors, faulty
fuel pump or
pump circuit), contaminated or old gasoline, defective throttle position sensor, faulty idle air control valve,
leaks in the EGR system, intake vacuum
leaks, and so forth.
There are several possible sources that trigger these two codes including a faulty
fuel pressure regulator, weak
fuel pump, clogged
fuel filter, faulty powertrain control module, vacuum
leak, faulty injector (s), faulty oxygen sensor (s) and a faulty mass air...
Rough idle and excessive shaking of the engine could be due to a bad tank of gasoline, dirty
fuel injectors, incorrect idle speed, exhaust restrictions, vacuum
leaks, incorrectly installed or damaged spark plugs, a defective or clogged
fuel pump or
fuel filter, failing electrical components such as the ignition control module, plug wires, coils, and spark plugs, a defective airflow sensor, dirty oxygen sensors and other causes.
However, if all cylinders are affected, you would test, in turn: the
fuel pressure regulator,
fuel pump pressure, check for a clogged
fuel filter, vacuum
leak, faulty oxygen sensor (s) and faulty mass air flow sensor.
If there is no
leaks, then go to the fuse box under the hood and locate the
fuel pump relay.
The poor operation that you are describing could be due to a bad tank of gasoline, dirty
fuel injectors, incorrect idle speed, exhaust restrictions, vacuum
leaks, incorrectly installed or damaged spark plugs, a defective or clogged
fuel pump or

GM says the flange can melt causing the
pump to
leak fuel, leading to a fire hazard.
The lack of response to the accelerator pedal has quite a few potential causes that would have to be sorted out in a diagnostic including, just as a few examples, plugged
fuel injectors, faulty oxygen sensors,
fuel pump and / or filter, vacuum
leaks including that caused by a faulty EGR system, ignition system malfunctions, bad valve timing if the timing belt or chain is worn or skipped, blocked exhaust (e.g., catalytic converter fails or becomes clogged), a faulty airflow sensor or throttle position sensor, and so forth.
Loss of power on acceleration can be due to plugged
fuel injectors, faulty oxygen sensors,
fuel pump and / or filter, vacuum
leaks including that caused by a faulty EGR system, ignition system malfunctions, bad valve timing if the timing belt or...
The causes include vacuum
leaks and EGR system faults, a faulty idle air control (IAC) valve or circuit, faults in the
fuel pump control circuit, defective...

In the future, when fossil
fuels are no longer the leading source of energy around the world and oil platforms aren't
pumping oil from beneath the ocean floor, we won't have to worry about oil spills, but unfortunately, ocean pollution from spills,
leaks and other sources is still a reality.
(For gas midstream projects the emissions may be more significant, due to
leaks from pipelines and
pump stations, as well as by consumption of
fuel used to liquefy the natural gas for marine transport).
