Application and Veterinary Care Form (completed
by a
licensed veterinarian).
A priority for accepting an
application for funding is a life - saving emergency confirmed
by a
licensed veterinarian either
by fax or phone.
A valid certificate of rabies inoculation issued
by a
licensed veterinarian must accompany your
license application.
Any person who submits an
application prior to January first, nineteen hundred eighty - three and who submits evidence of employment
