Since the early 1990s, government policy on maternity care in England has moved towards policies designed to give women with straightforward pregnancies a choice of settings for birth.1 2 In this context, freestanding midwifery units, midwifery units located in the same building or on the same site
as an obstetric unit (hereafter referred to
as alongside midwifery units), and home birth services have increasingly become relevant to the configuration of maternity services under consideration in England.3 The
relative benefits and risks of birth in these alternative settings have been widely debated in recent years.4 5 6 7 8 9 10 Lower rates of obstetric interventions and other positive maternal
outcomes have been consistently found in planned births at home and in midwifery units, but clear conclusions regarding perinatal
outcome have been lacking.
We also estimated
relative indices of inequality (RII) and slope indices of inequality (SII)
as summary measures of

outcomes, respectively, across the entire distribution of maternal education.24 For child IQ, linear regression analyses using GEEs were performed to estimate mean IQ differences in lower maternal education from the reference category in each intervention group and compared between the groups.

as a necessary precursor to identifying whether there are better ways to provide some services.

outcomes, including percentage who received well - child visits and dental service.25 — 27 HFA had favorable results for 4 health care

relative to what was observed in the early phase of the UK Sure Start evaluation.3 The current results are also comparable in size to those found in the later impact evaluation of the Sure Start programme, in which 3 - year - old children were exposed to mature SSLP throughout their entire lives.4 Reviews of the effectiveness of early childhood interventions have found that most studies reported effect sizes on parenting and child
outcomes that were small to moderate.14 15 When comparing CfC and SSLP with other interventions, it is important to remember that the evaluations of these interventions measured effects on an entire population, rather than on programme participants,
as is the case in the evaluation of many other interventions.
We found no associations between child involvement and other treatment variables, however, it is not entirely clear what role child and parent involvement played and it would be useful for studies to look at the

Consistent with prior short - term
outcomes, the primary measure at age 3 years was externalising behaviour problems, assessed by the 99 - item validated Child Behaviour Checklist (CBCL 1 1/2 — 5).21 This checklist also quantifies internalising behaviour problems, and yields raw scores (used to compare groups
