Permit applications must include
basic information that should be readily available to the importer: The name and address
of the person intending to export the dog; the name and address
of the importer; the number
of dogs to be imported and their breed, sex, age, color, markings, and other identifying information; the purpose
of the importation; the port
of embarkation and
mode of transportation; the port
of entry in the United States; the proposed date
of arrival in the continental United States or Hawaii; the name and address
of the person who will take delivery
of the dogs; and, if the dogs will be used for research, the USDA registration number
of the research facility.
