If your proofreader finds a mistake, he or she has to pass it to the book interior designer to fix those mistakes and ensure the changes don't negatively affect the overall layout.
It also makes it so that if you have a change to your book's cover or interior, you don't have to recycle a bunch of already printed copies you can no longer sell; you can simply update your file and every copy sold and printed thereafter will reflect the change.
Plus, every once and a while I go through and want to change something I wrote, so I can easily update the interior file and «fix» whatever I didn't like.

Things like image formatting / image placement (the issue with images is that it fouls things up when it comes to saving / navigating a document along with editing comments and track changes), text boxes, symbols, graphs, interior layout, front cover, and other stylistic measures should be added only after the bulk of the narrative is secure and fairly polished (read this: the editor has done most of the heavy lifting of the edit).
