Recent work: new fuel and air filters, spark plugs, wires, and distributor cap, PCV valve, replaced intake manifold gaskets, catalytic converter,
cleaned MAF sensor, and replaced PCM.

you cleaned your MAF?
I've just
cleaned the MAF to check if this would help but I'm still having the issue.
I cleaned MAF and changed fuel filter, but no change.
I just remembered this was a question - re-charging the battery, changing out the spark plugs, and
cleaning the MAF fixed this.
I'd start by checking / cleaning / replacing the intake air filter, then
cleaning the MAF with electronic safe cleaner (can be found at auto parts stores).
Cleaning the MAF sensor with electronic cleaner may help restore the sensor's functionality.
There's many videos on how to
clean a MAF, and some people say you can clean it with acetone, I'd suggest a can of MAF cleaner spray, and maybe a gentle brush - down with a soft paintbrush.
If it works it will save you 50 USD for the part, if not you've spent a few bucks on a can of cleaner you'll use anyway to regularly
clean the MAF, which is recommended.
Clears all trouble codes and completes a test drive to see if the P2281 diagnostic trouble code comes back (if so, checks the MAF sensor for dirt and debris and
clean the MAF sensor as necessary)

CHRA, valve cover,
cleaned intake valves and replaced intake gasket, 2 cans of BG44K currently, swapped front o2 sensor and
MAF with no change.
I spent about 300 bucks on plugs, wires, cap an rotor, ignition coil, fuel filter, air filter, vacuum lines and some
cleaning sprays (
maf cleaner and tb
cleaner).
MAF cleaner is US$ 10, tops.
Removed the
MAF sensor and got it
cleaned with some spray as little dust was accumulated in it (carburetor spray i believe)
Clean the Mass Air Flow sensor with
MAF cleaner (that is very sensitive to the solvents, make sure you are using real
MAF cleaner) If it still stalls, check fuel filter, replace if it hasn't been replaced in your maintenance window.
but i replaced both AirFilter and
MAF sensor (original sensor was 4pin but replaced one is a 5pin) and i asked mechanic to tune the engine, as there is nothing to tun in EFI... he
cleaned the engine.
Since your cylinders may be especially dirty from running without the
MAF sensor, I'd recommend using seafoam to
clean it up.
UPDATE I didn't realize it but the dirty
MAF really did add hesitation to the engine, the gas pedal is a lot more snappy after this
cleaning and I'm pretty sure the mileage has gone up a few mpg.
So far so good with the
MAF cleaning, haven't had the codes pop up for about 850mi post
cleaning and the
MAF housing's interior and the laminar air screen were definitely dirty.
I read some pretty good reports regarding
MAF cleaner, so I figured I'd give it a shot.
If a
MAF clean / replacement doesn't resolve the symptoms then another potential cause (based on the symptoms you describe) would be the Electronic Throttle Module (ETM), these were problematic on the T5 engine (especially on early 2000's models) to the extent that Volvo extended the warranty on them if I recall correctly.
While I couldn't agree more with
cleaning the heating element in the
MAF, using electronic or
MAF cleaner would be a much better way to go.
There is the possibility that your
MAF might be dirty - You can also try

MAF Cleaning, IAC, Intake manifold gasket (he did not change the valve seals), temp sensor.
The
maf and iMac have been
cleaned.
Carb
cleaner leaves an oily type residue that is not good for the electronics within the
MAF sensor.
