Along the way, I've received support from
community health nurses,
community health care
professionals, rape relief and
women's shelters, police, doctors, a counsellor, school / daycare teachers and lots and lots
of friends and
community members.
And not only to IBCLCs as there are other kinds
of professional lactation supporters available to
women - Certified Lactation Counselors, Advanced Lactation Consultants and Advanced Nurse Lactation Consultants are also available in many
communities - and they are sometimes more numerous in number than IBCLCs.
This is a unique opportunity for birth
professionals of all kinds to deepen your knowledge and skills in holistic health while also being part
of a nurturing
community of dynamic
women.
Although
professional lactation support can improve the duration
of overall breast feeding, its effect in improving exclusive breast feeding is unclear.11 18 22 Thus far, studies that report improvement
of rates
of exclusive breastfeeding have involved mainly
community based peer counselling strategies.23 24 25 Even then, a randomised trial in the UK recently cast doubt on the efficacy
of this approach.26 There are current recommendations from NICE for the UK - wide implementation
of the baby friendly initiative.4 5 6 The 2006 NICE costing report on routine postnatal care
of women and their babies estimates that efforts to improve rates
of breast feeding will result in substantial cost savings for the NHS.6
Other models
of care include a) where the physician / obstetrician is the lead
professional, and midwives and / or nurses provide intrapartum care and in - hospital postpartum care under medical supervision; b) shared care, where the lead
professional changes depending on whether the
woman is pregnant, in labour or has given birth, and on whether the care is given in the hospital, birth centre (free standing or integrated) or in
community setting (s); and c) where the majority
of care is provided by physicians or obstetricians.
