To obtain a duplicate certificate, contact the public school or driver education school that you attended or contact your parent
taught driver education course provider, if you completed the course via the Parent Taught Driver Education Program.

By law, approved course providers are required to keep records for a number of years, so you may be able to get a replacement copy from
your driver education course provider.
If you have lost your DE-964 certificate and need a replacement, contact
your driver education course provider.
The legislation (SB 848 / HB 912) removed outdated language, expanded the group of persons who are authorized to provide instruction in the «parent taught» driver education program, reduced
the driver education course provider bond to $ 10,000 and authorized driver education and driving safety course completion certificates to be issued electronically.
You must show proof by presenting a valid Certificate of Driver Education enrollment form from
the driver education course provider.
