How best to structure these is an open question; recent findings from Rites of Passage, an
early intervention programme for Aboriginal young people which
includes resilience - building camps and increasing access to mental health services, suggest that boys may be more difficult than girls to engage in social and emotional well - being programmes (Robinson R, Schuster L, Williamson A. Rites of Passage: evaluation if a pilot study if an
early intervention program for Aboriginal young people.
The reasoning behind this proposition is that: A) EBHV
programs are designed to serve women categorized as «at - risk» due to a variety of demographic factors,
including single - parent household status, age at time of first pregnancy, being categorically undereducated, under or unemployed, and meeting federal standards of living at or below the poverty line; B) these
programs serve women during pregnancy and / or shortly after the birth of their children, offering an excellent chance for the
early prevention of trauma exposure; and C)
intervention services are provided at the same times that attachment (whether secure or insecure) is being developed between mothers and children, providing the opportunity that generational risk may be mitigated.
Program evaluation has supported this multifaceted approach in multiple countries and settings.83 Analyses by Nobel Prize — winning economist James Heckman reveal that
early prevention activities targeted toward disadvantaged children have high rates of economic returns, much higher than remediation efforts later in childhood or adult life.84 For example, the Perry Preschool
Program showed an average rate of return of $ 8.74 for every dollar invested in
early childhood education.85 Targeted
interventions foster protective factors,
including responsive, nurturing, cognitively stimulating, consistent, and stable parenting by either birth parents or other consistent adults.
Specific
programs (
including not only health care but also maternal — infant child home visiting and
early intervention) offer crucial assistance to families with young children.
In the Elmira demonstration,
intervention mothers were less likely to punish or physically restrain their children than mothers in the control group.62 Among home - visited families who participated in
Early Start, less punitive parenting was observed, though the effect was modest.63 Several other
programs have identified reductions in the frequency with which mothers spanked their children at thirty - six months,
including Healthy Families San Diego, 64
Early Head Start, 65 and IHDP.66 No effects on harsh parenting were found in the CCDP.67
In another evaluation of an
early -
intervention approach, parents of 51 preschool - aged children suspected of having an ASD participated in the Hanen More Than Words
program either immediately or after a delay.15 Investigators» operationalization of «suspected ASD»
included identification of language delay and concerns about social behavior by a pediatrician and / or a speech and language therapist, which resulted in inclusion of children without ASDs within the
intervention and control groups.
One Colorado study showed that paraprofessional home visiting, when combined with an
early -
intervention program focused on children with developmental delays, resulted in improved involvement with the
program.25 In North Carolina, the combination of a public health department's home - visiting
program with links into private physician's offices was helpful in overcoming personal and structural barriers to care.43 The Commonwealth Fund's Healthy Steps
intervention included home visiting by masters - level healthy development specialists with significant gains in the quality of well - child care, although the multifactorial nature of this
intervention made it difficult to evaluate the effectiveness of the home - visiting component.44 — 46 A South Carolina study showed that a
program that linked school - based home visitors to group well - child visits resulted in greater retention of anticipatory guidance and improved satisfaction with care.47
Further, few studies have reported economic evaluations for
early intervention programs for children's mental health.7, 25,32 Such evaluations could
include implementation service costs (training,
program materials, provider salaries), costs to families (time off work, transport costs), and later health / welfare costs saved from implementing an
early intervention.
