Physical punishment is associated with a range
of mental health problems in children, youth and adults, including depression, unhappiness, anxiety, feelings
of hopelessness, use
of drugs and
alcohol, and general psychological maladjustment.26 — 29 These relationships may be mediated by disruptions in parent — child attachment resulting from pain inflicted by a caregiver, 30,31 by increased levels
of cortisol32 or by chemical disruption
of the brain's mechanism for regulating stress.33 Researchers are also
finding that physical punishment is linked to slower cognitive development and adversely affects academic achievement.34 These
findings come from large longitudinal studies that control for a wide range
of potential confounders.35 Intriguing results are now emerging from neuroimaging studies, which suggest that physical punishment may reduce the volume
of the brain's grey matter in areas associated with performance on the Wechsler Adult Intelligence Scale, third edition (WAIS - III).36 In addition, physical punishment can cause alterations in the dopaminergic regions associated with vulnerability to the
abuse of drugs and
alcohol.37

Findings from the NSCAW indicate that substance
abuse was much more highly associated with «neglect, failure to provide basic necessities» than with «neglect, failure to supervise» or any type
of abuse.11 Finally, violence may be more likely to erupt in homes where stimulant drugs and
alcohol are used.12 The interplay between substance
abuse and child maltreatment within family dynamics and across children's developmental periods is gradually becoming clearer.
These
findings approximate those
of the more recent National Survey
of Child and Adolescent Well - Being (NSCAW) that 20 percent
of children in an investigation for
abuse and neglect had a mother who, by either the child welfare worker's or mother's account, was involved with drugs or
alcohol; that figure rises to 42 percent for children who are placed into foster care.7 These studies have clearly established a positive relationship between a caregiver's substance
abuse and child maltreatment among children in out -
of - home care and among children in the general population.
API children have been
found to have lower adjusted odds
of any mental health service use, outpatient mental health service use, and 24 - hour - care service use (ie, inpatient, residential, group - home, or
alcohol / drug
abuse treatment)(Table 2).
Substance
abuse Substance
abuse by a child's parent or guardian is commonly considered to be responsible for a substantial proportion
of child maltreatment reported to the child welfare services.1 Studies examining the prevalence
of substance
abuse among caregivers who have maltreated their children have
found rates ranging from 19 percent2 to 79 percent or higher.3 One widely quoted estimate
of the prevalence
of substance
abuse among care - givers involved in child welfare is 40 to 80 percent.4 An epidemiological study published in the American Journal
of Public Health in 1994
found 40 percent
of parents who had physically
abused their child and 56 percent who had neglected their child met lifetime criteria for an
alcohol or drug disorder.5
A review
of twenty studies on the adult lives
of antisocial adolescent girls
found higher mortality rates, a variety
of psychiatric problems, dysfunctional and violent relationships, poor educational achievement, and less stable work histories than among non-delinquent girls.23 Chronic problem behavior during childhood has been linked with
alcohol and drug
abuse in adulthood, as well as with other mental health problems and disorders, such as emotional disturbance and depression.24 David Hawkins, Richard Catalano, and Janet Miller have shown a similar link between conduct disorder among girls and adult substance
abuse.25 Terrie Moffitt and several colleagues
found that girls diagnosed with conduct disorder were more likely as adults to suffer from a wide variety
of problems than girls without such a diagnosis.26 Among the problems were poorer physical health and more symptoms
of mental illness, reliance on social assistance, and victimization by, as well as violence toward, partners.

Several studies
of inpatients have noted that depressed individuals are significantly less likely to be violent than individuals with other types
of disorders.8 - 10, 23 Moreover, affective disorders were not
found to be related to an increased risk for homicide in a Finnish cohort study.15 By contrast, other studies have
found a relationship between affective disorders and homicide, 24 self - reported violent behavior, 2 and conduct disorder in childhood and adolescence.25 One possible explanation for these conflicting results could be the potential moderating role
of alcohol abuse in this relationship.

For example, longitudinal prospective studies
of children
of mothers with schizophrenia have consistently identified a subgroup who present a stable pattern
of aggressive and / or antisocial behavior from a young age, 36,37 and studies
of children with depression identify a subgroup with conduct disorder.38 Among persons who develop a major mental disorder, substance
abuse in childhood or adolescence is more strongly associated with violent crime in adulthood than substance
abuse in adulthood, 39 and individuals who will develop a major mental disorder may be more sensitive to the effects
of alcohol than others.40 These
findings suggest that symptoms (eg, substance
abuse) that we and others are labeling as distinct disorders, may in fact be a part
of or at least related to the primary disorder.
