Felitti
and colleagues1 first described ACEs
and defined it as exposure to psychological, physical or sexual
abuse,
and household dysfunction including substance
abuse (problem drinking / alcoholic
and / or street drugs), mental illness, a mother treated violently
and criminal behaviour in the household.1 Along with the initial ACE study, other studies have characterised ACEs as
neglect, parental separation, loss
of family members or friends, long - term financial adversity
and witness to violence.2 3 From the original cohort
of 9508 American adults, more than half
of respondents (52 %) experienced at least one adverse childhood event.1 Since the original cohort, ACE exposures have been investigated globally revealing comparable prevalence to the original cohort.4 5 More recently in 2014, a survey
of 4000 American children found that 60.8 %
of children had at least one
form of direct experience
of violence, crime or
abuse.6 The ACE study precipitated interest in the health conditions
of adults maltreated as children as it revealed links to chronic diseases such as obesity, autoimmune diseases, heart, lung
and liver diseases,
and cancer in adulthood.1 Since then, further evidence has revealed relationships between ACEs
and physical
and mental health outcomes, such as increased risk
of substance
abuse, suicide
and premature mortality.4 7
Rutter & Quinton (1977) found that factors existing in children's social environment were linked to health - risk behaviors later in life,
and were the first researchers to describe
neglect,
abuse,
and other
forms of maltreatment (what would later be considered adverse childhood experiences, or ACEs) in terms
of their cumulative effect, range
of adversity,
and wide - reaching impact on both mental
and physical health over the course
of an individual's lifetime.
Sadly, the research establishes that there is a high probability
of intergenerational transmission
of childhood trauma; this has been found to be true for a variety
of forms of trauma (e.g.,
neglect, physical
and sexual
abuse, domestic violence, substance
abuse) in numerous samples
of parents
and children (e.g., Langeland & Dijkstra, 1995; Pears & Capaldi, 2001, Stith et al., 2000; Widom, Czaja, & DuMont, 2015).
Indeed, Jay Belsky incorporated all
of these risk factors into his process model
of parenting, 11
and data from multiple studies support links to child well - being.12 In an experiment on the effectiveness
of a program for low - birth - weight infants, Lawrence Berger
and Jeanne Brooks - Gunn examined the relative effect
of both socioeconomic status
and parenting on child
abuse and neglect (as measured by ratings
of health providers who saw children in the treatment
and control groups six times over the first three years
of life, not by review
of administrative data)
and found that both factors contributed significantly
and uniquely to the likelihood that a family was perceived to engage in some
form of child maltreatment.13 The link between parenting behaviors
and child maltreatment suggests that interventions that promote positive parenting behaviors would also contribute to lower rates
of child maltreatment among families served.
