Transitional care (first 4 — 8 hours): The infant should be kept warm during a physical
assessment for
gestational age, intrauterine growth status, temperature, heart and respiratory rates, skin color, peripheral circulation, respiration, consciousness, and tone, and should be monitored every 30 minutes for 2 hours.
