Using a public
health frame, we will examine how three evidence - based home visiting
models form a continuum of interventions directly addressing this challenge: (1) Family Connects provides nearly universal assessment of needs for families of newborns, with connection to community services (Karen O'Donnell, Duke University); (2) Healthy Families America focuses on prevention through facilitating nurturing relationships and connection to services (Kathleen Strader, Healthy Families America); and (3) Child First targets the most vulnerable young children and families, who have experienced high levels of trauma and adversity, through a team approach providing comprehensive
care coordination and mental
health intervention for both parent and child (Darcy Lowell, Child First).
Data for the implementation and impact studies will be collected from a variety of sources, including interviews with parents; observations of the home environment; observed interactions of parents and children; direct assessments of children's development; observations of home visitors in their work with families during home visits; logs, observations, and interviews with home visitors, supervisors, and program administrators; program
model documentation from program developers, grantees, and local sites; and administrative data on child abuse,
health care use, maternal
health, birth outcomes, and employment and earnings.

care using the ISOQOL, minimum standards for patient - reported outcome measures (conceptual and measurement
model, reliability, content validity, construct validity, responsiveness, interpretability of scores, translation of measure, patient and investigator burden), 43 and the CREATE checklist (descriptive system,
health states values, sampling, preference data collection, study sample,
modelling, scoring algorithm).44 The ISOQOL minimum standards were chosen as these standards were developed from a systematic review of published and unpublished guidance on patient - reported outcome measures, including the COnsensus - based Standards for the selection of
health Measurement Instruments (COSMIN).46 To the authors» knowledge, the CREATE checklist is the only published guidance on what key components should be reported in a valuation study.44 Information on how the contents of the instruments were developed, psychometric properties and valuation will be
used to assess the suitability of the instruments for the palliative setting; instruments will be scored on whether the domains or dimensions were developed
using input from informal caregivers of people receiving palliative
care (yes / no) and whether each of the reporting checklist items has been evaluated for this population (if yes, then a score of one will be allocated) and a total score calculated.

Specifically, the ACE Study
model relies strongly on the idea that adverse childhood experiences create a burden of psychological stress that changes behavior, cognitions, emotions, and physical functions in ways that promote subsequent
health problems and illness.22 Among the hypothesized pathways, adverse childhood experiences lead to depression and posttraumatic stress disorder, which in turn can lead to substance abuse, sleep disorders, inactivity, immunosuppression, inflammatory responses, and inconsistent
health care use, possibly leading to other medical conditions later in life.23, 24 Therefore, childhood behavioral and emotional symptoms very likely represent a crucial mediator linking adverse childhood experiences and the longer term
health - related problems found in the ACE substudies.
