requested documentation from the Department of
Health and Human Services and the Department of Agriculture about the benefits of flossing.
Students who would like to
request extended time related to a disability, medical / mental
health issues, or English language proficiency, must provide
documentation.

documentation, statements, or representations from the person
requesting the protected
health information that is required as a condition of the disclosure.
(ii) Obtain any
documentation, statements, or representations, whether oral or written, from the person
requesting the protected
health information when such
documentation, statement, or representation is a condition of the disclosure under this subpart.
Where
documentation, statements or representations, whether oral or written, from the person
requesting the protected
health information is a condition of disclosure under this rule or other law, this verification must involve obtaining such
documentation statement, or representation.
If a disclosure is conditioned by this subpart on particular
documentation, statements, or representations from the person
requesting the protected
health information, a covered entity may rely, if such reliance is reasonable under the circumstances, on
documentation, statements, or representations that, on their face, meet the applicable requirements.
If the individual provides insufficient
documentation and does not provide the missing information in a timely manner after the employer's subsequent
request, the employer may require the individual to go to an appropriate
health professional of the employer's choice.
You should also use the time to gather any medical
documentation, such as your
health records, in case more information is
requested by the disability insurance company.
A

documentation of vital signs.
