The strengths of the study
include the ability to compare outcomes by the

labour, the high participation of midwifery units and trusts in England, the large sample size and statistical power to detect clinically important differences in adverse perinatal outcomes, the minimisation of selection bias through achievement of a high response rate and absence of self selection bias due to non-consent, the ability to compare groups that were similar in terms of identified clinical risk (according to current clinical guidelines) and to further increase the comparability of the groups by conducting an additional analysis restricted to
women with no complicating conditions identified at the start of care in
labour, and the ability to control for several important potential confounders.
Among 64 538 low - risk
women, of whom more than 16 000 planned a homebirth at the onset of
labour, no difference was found in the adjusted odds between obstetric units and other birthplaces,
including homebirth.

The study population
included all «low risk»
women who participated in the cohort study, as described elsewhere.12 In brief, the cohort study was designed to compare outcomes in
women judged to be at low risk of complications before the onset of
labour.

includes essential care during childbirth and in the postnatal period for every mother and baby,
including antenatal steroid injections (given to pregnant
women at risk of preterm

included.
The midwife - led continuity model of care
includes: continuity of care; monitoring the physical, psychological, spiritual and social well being of the
woman and family throughout the childbearing cycle; providing the
woman with individualised education, counselling and antenatal care; attendance during
labour, birth and the immediate postpartum period by a known midwife; ongoing support during the postnatal period; minimising unnecessary technological interventions; and identifying, referring and co-ordinating care for
women who require obstetric or other specialist attention.
Other tools in the toolkit that midwives also referred to
included ensuring the
woman has adequate nutrition and hydration in
labour, keeping the
woman active, and homeopathy and hypnotherapy to help the
woman to relax and to reduce anxiety.
Women's experiences of care reported in the original studies
include maternal satisfaction with information, advice, explanation, venue of delivery and preparation for
labour and birth, as well as perceptions of choice for pain relief and evaluations of carers behaviour.
We searched PubMed, Embase, and Cochrane databases for randomised controlled trials of maintenance tocolysis therapy with nifedipine in preterm
labour.We selected trials
including pregnant
women between 24 and 36 (6/7) weeks of gestation (gestational age, GA) with imminent preterm
labour who had not delivered after 48hours of initial tocolysis, and compared maintenance nifedipine tocolysis with placebo / no treatment.The primary outcome was perinatal mortality.
These
included clinical assessment skills in the antenatal period, documentation of instructions given to clients to ensure that midwives could arrive at the birth in a timely fashion, early transport after identification of thick meconium in the amniotic fluid and active management of the third stage of
labour for
women at risk for postpartum hemorrhage.11
Compared with
women who planned a hospital birth with a midwife or physician in attendance, those who planned a home birth were significantly less likely to experience any of the obstetric interventions we assessed,
including electronic fetal monitoring, augmentation of
labour, assisted vaginal delivery, cesarean delivery and episiotomy (Table 3).
In the study group, we
included all births in British Columbia between Jan. 1, 2000, and Dec. 31, 2004, that were planned to take place at the
woman's home at the onset of
labour.
In a second subgroup analysis, we
included only
women whose
labour was spontaneous.

labour or has given birth, and on whether the care is given in the hospital, birth centre (free standing or integrated) or in community setting (s); and c) where the majority of care is provided by physicians or obstetricians.
Labour has focused on the fact that recent departures from senior quango positions -
including the chairs of English Heritage, the Charities Commission and the Arts Council - have all been
women.
The Prime Minister claims she has raised issues
including women's rights with the Saudi government but the
Labour leader says she must go further and ban arms sales to the regime.
