The amount of
lactulose used by us is far below the 20 g
lactulose used by Ropert et al (13) to affect proximal gut motility and gastric tone, but Piche et al (14) obtained more relaxation of the lower esophageal sphincter with only 6.6 g fructooligosaccharide, a substrate similar to lactulose in its rate and extent of fermentation.

Hepatic encephalopathy occurs when the liver can not remove certain toxins and chemicals, such as ammonia, from the blood.1 These toxins and chemicals then build up and enter the brain.1 Hepatic encephalopathy is one of the major complications of cirrhosis (scarring of the liver), and a leading cause of hospital re-admission due to its recurrence, despite treatment.1 It can occur suddenly in people with acute liver failure, but is seen more often in those with chronic liver disease.1 Symptoms of hepatic encephalopathy include mild confusion, forgetfulness, poor concentration and personality or mood changes, but can progress to extreme anxiety, seizures, severe confusion, jumbled and slurred speech and slow movement.1 The first step in treatment is to identify and treat any factors that cause hepatic encephalopathy.2 Once the episode has resolved, further treatment aims to reduce the production and absorption of toxins, such as ammonia.1 Generally, there are two types of medication
used to reduce the likelihood of another hepatic encephalopathy episode —
lactulose and rifaximin.2 However, it remains a leading cause of hospitalisations and re-hospitalisations in cirrhotic patients, despite the
use of the above - mentioned standard of care treatment.
Using a technique referred to as
lactulose hydrogen breath testing (LHBT), researchers were able to gauge concentrations of bacteria in the bowels of nearly 200 participants.

lactulose breath test

The purified fiber
used for the LGI breakfast and for 1 of the 2 HGI breakfasts was a preparation of nonfermentable purified cellulose from hazelnut shells (ALSO, Zelbio, Italy), whereas, in the second HGI breakfast, the HGI - Lac meal, cellulose was replaced with 5 g of the undigestible and highly fermentable disaccharide
lactulose in crystalline form (INALCO, Milan, Italy).
Patients who weigh more than 25 pounds will
use all 15 mL of the liquid (10 grams of
lactulose).

uses glucose as the testing substrate and one that
uses lactulose as the substrate.

Lactulose, a non-absorbable sugar, is one of the most widely
used medications to lessen the symptoms of hepatic encephalopathy.
Petrolatum - based remedies must not be over-
used and
lactulose must be
used cautiously in diabetic cats.

If the tests that your vet
used to diagnose the condition improve while on
lactulose, that is certainly reason enough to continue giving it.
