I'd bet that the Pratchett / Snuff typo was the result of using an optional hyphenation code or a
non-breaking space or something equally silly; installed by a lazy typesetter to fix a run in a dead tree edition, missed when generating the output epub because it was (a) standards compliant (i.e. used the right
character codeset), (b) invisible to the naked eye when previewing the DTP file on screen, and (c) the epub reader they looked at the file in didn't have the same bug as Amazon's file filter and displayed it correctly.
