Unlike adult studies in which medication rates were higher and increased with

intervention did not increase medication rates, despite
intervention support
of medication treatment.
At 6 - month follow - up, patients receiving the
quality improvement intervention reported significantly higher rates
of mental health care than did those receiving usual care (32 % vs 17 %; OR, 2.8; 95 % CI, 1.6 to 4.9; P <.001)(Table 2).
This
improvement among
intervention patients was also reflected in a significantly lower rate
of severe depression (CES - D score ≥ 24) in the
quality improvement group (31 % vs 42 %; OR, 0.6; 95 % CI, 0.4 to 0.9; P =.02).
The
quality improvement intervention included (1) expert leader teams at each site that adapted and implemented the
intervention; (2) care managers who supported primary care clinicians with patient evaluation, education, medication and psychosocial treatment, and linkage with specialty mental health services; (3) training
of care managers in manualized CBT for depression; and (4) patient and clinician choice
of treatment modalities (CBT, medication, combined CBT and medication, care manager follow - up, or referral).
Building on prior demonstrations
of improved outcomes from
quality improvement interventions for adult and late - life depression, 52,63 our results indicate that this approach can be adapted successfully for younger populations with similar outcomes.
In conclusion, the present results demonstrate that
quality improvement interventions for adolescent depression are feasible in primary care settings and associated with benefits on measures
of depression,
quality of life, and satisfaction with mental health treatment.
This is the first demonstration that depression and
quality -
of - life outcomes can be improved through a
quality improvement intervention for depressed adolescents in primary care settings.
Lack
of responsive, contingent, and sensitive care is one
of the most prominent deviations from the expected environment for institutionalized children, and foster care placement in BEIP is associated with dramatic
improvements in caregiving
quality and attachment security (41, 42), both
of which might contribute to the
intervention effects on stress response system functioning.
These findings demonstrating enhanced child and parent benefits associated with collaborative care extend those reported in
quality improvement interventions for child behavior problems, 13,14 ADHD, 6 — 8 adolescent depression, 10,11 and other problems.5 In the follow - up period, EUC showed significantly greater remission since posttreatment in behavior problems than DOCC, which may reflect DOCC patients having achieved greater remission by the end
of treatment.
Research into the mechanisms
of change between parenting
interventions and couple relationship
quality suggests that
improvements in parenting skills and child behaviour lead to better relationships between parents.
The
quality improvement intervention included: expert leader teams at each site to implement and adapt
intervention; care managers to support primary care clinicians with evaluation, education, medication, and psychosocial treatment, and linking with specialised mental health services; training care managers in manual cognitive behaviour therapy (CBT) for depression; and access to participant and clinician choice
of treatment (CBT, medication, combined CBT and medication, care manager follow up, or referral).
In contrast, a randomized, controlled trial
of a practice - based
quality improvement initiative in North Carolina led to small changes in receipt
of anticipatory guidance (only 18 %
of parents
of 6 - month - old children in the
intervention group discussed all 4 anticipatory guidance topics) and no changes in parent behaviors.30
Although Healthy Steps was not conceived as a «
quality improvement»
intervention, our findings indicate that Healthy Steps made its impact through
improvements in
quality of care and selected effects on parenting practices.
In adolescents with depression,
quality improvement intervention reduced depression, and increased mental health
quality of life and satisfaction with mental health care compared with usual care (see http://www.ebmentalhealth.com/supplemental for table).

interventions to promote child development globally, using a combination
of experimental, ethnographic and

improvement during drug therapy or relevant psychosocial impairments, cognitive behavioral as well as mindfulness and acceptance - based
interventions can improve

of the affected patients.
Although no data are currently available on the effects
of this
intervention for child attachment, it is nonetheless the case that preliminary analyses show an

of parental sensitivity, shared enjoyment and the children's increased ability to govern their emotions.
S. Department
of Health and Human Services launched the national
Quality Improvement Center on Domestic Violence in Child Welfare (QIC - DVCW) to test
interventions to improve how child welfare agencies and their partners work with families experiencing domestic violence.
Specifically, routine data and a systematic literature review will inform specific parameters that will link early
interventions on infant mental health — and related
improvements in
quality of life — with better health, educational and occupational outcomes, lower crime rates and cost savings for the health sector and the entire society.
Results indicated higher
quality friendships and more teacher - perceived
improvement for children whose buddies were lower on antisocial behavior and whose parents were higher on compliance; however, corresponding
improvements as perceived by counselors and parents were not obtained.50 These preliminary results suggest the need for more research examining the potential
of friendship
interventions and the extent to which their success may relate to the characteristics
of the children involved in the friendship.
Although
improvements in
quality of life were marginal, because
of health service costs avoided the
intervention had a 70 % chance
of being cost - effective, with a baseline cost per QALY gained
of $ 6827 (2003 prices).
