We require a
completed owner surrender form which provides us with information about your dog and states that you have released your animal into our care for adoption to a new home.
If your pet is suffering and has a terminal illness, please complete the
online owner surrender form, linked below, and explain your pet's disease and condition.
The animal according to the law is now their animal and
an owner surrender form will need to be completed and signed and a fee will apply.
Print and complete
our Owner Surrender Form and bring it with you when you come to the Animal Service Center with your pet.
If we are able to find a foster home for your pet, we will contact you then ask you to complete
the owner surrender form.
If you adopted your pet from us and need to return him / her, complete
the Owner Surrender Form and indicate that your pet was adopted from The Animal League in the notes section.
There is
an owner surrender form; and there is a surrender fee to help offset the cost of caring for the cat going forward.
In order for us to consider your surrender, please complete
the owner surrender form and tell us about your companion.
You will be asked to complete two
owner surrender forms.
The second is
the Owner Surrender Form.
If you have a purebred German Shepherd that is seeking a new home, please complete
the Owner Surrender form to tell us more about your situation and your dog.

At this time, you will be asked to sign
the owner surrender form, with which you transfer all ownership rights to East Bay SPCA.
Fill out
an owner surrender form which will be e-mailed to you upon acceptance of your dog in to the rescue - we will need this when picking up the dog.
If you need to surrender your pet, please fill out
the owner surrender form and a volunteer will contact you.
If you are in need of assistance rehoming your Shar - Pei or finding help for a found stray, please complete

We do not take any cats into our foster system without
an Owner Surrender form.
If we are able to accommodate your cat you will need to fill out and sign
an Owner Surrender form.
Please understand that we will also need pictures of your dog, your vet's contact information and
an owner surrender form filled out prior to being able to accept the dog into rescue.
