Psychotherapy notes refer to confidential notes taken by a therapist or counselor during a therapy session. They include observations, thoughts, and discussions about a person's mental health. They are separate from regular medical records and are meant to help the therapist provide effective treatment.

It says that an individual is entitled to a copy of protected health information (essentially anything with a name or identifying number on it)
except psychotherapy notes.
It is not River Ridge practice to keep
psychotherapy notes as defined by HIPAA, or to keep any client notes separate from the client's file.
The Privacy Rule requires covered entities to provide individuals with access to their medical records; however, the Privacy Rule
exempts psychotherapy notes from this requirement if they are separately maintained by the covered entity.
