A randomised controlled trial would be the best way to tackle selection bias
of mothers who plan a home birth, but a randomised controlled trial in North America is unfeasible given that even in Britain, where home birth has been an incorporated part
of the healthcare system for some time, and where cooperation is more feasible, a pilot
study failed.31 Prospective
cohort studies remain the most comprehensive instruments available.
Women in our
study had a high rate
of breastfeeding at 6 weeks postpartum (69 %) compared with other Australian data showing that 58 %
of all infants were fully breastfed at 2 months
of age.50 This may have been owing to a higher motivation
of women in our
cohort, and a good level
of support and continuity
of midwifery care, which has been shown to enhance rates
of breastfeeding.
