I suspect another large part of the distrust has to do with the fact that premies who are learning to feed don't consistently transfer the same
volume of milk even when their feeding quality seems to be subjectively «good.»

I think the spitting up more in the
evening is pretty normal — as is your lower
volume of milk.
In the normal course
of events, your
milk will change and increase in
volume even if you don't breastfeed your baby.
They
even had me feed the baby a minimal
volume of milk and when I said he was showing signs
of being full, they still made me give him some more because he hadn't had the quota recommended and sure enough he was sick, brought up most
of the feed and was upset.
This is important
even though you may only be producing a small
volume of milk right after delivery.
