caesarean, all excluding consultants» fees and
with # 1,000 for each extra 24 hours stayed.
In a randomised controlled trial comparing community based care
with standard hospital care a significant difference in
caesarean section rates was found (13.3 % v 17.8 % respectively).29 Planning a home birth30 or booking for care at a midwife led birth centre is also associated
with lower operative
delivery rates.
In Scotland, where wide variations in surgical
deliveries have been found between units, four evidence based recommendations have been prioritised: clinicians and women should regard trial of labour as the norm after a previous
caesarean; offering external cephalic version to women at term if their baby is breech; monitoring and regularly reviewing
caesarean data
with support for staff; and one to one midwifery care for all women in labour.20 The National Childbirth Trust — a UK parents organisation — is concerned about medicalisation and erosion of midwifery skills and confidence.
The extent of medicalisation in Spain is reflected in some of the highest
caesarean section rates in Europe (26.4 % in Catalonia
with a 40 % increase over five years); obstetricians have been criticised for not allowing women to participate in decisions about their maternity care.11 Long term morbidity after childbirth can be substantial, 12 and this is particularly related to instrumental and
caesarean delivery.
The odds of receiving individual interventions (augmentation, epidural or spinal analgesia, general anaesthesia, ventouse or forceps
delivery, intrapartum
caesarean section, episiotomy, active management of the third stage) were lower in all three non-obstetric unit settings,
with the greatest reductions seen for planned home and freestanding midwifery unit births (table 4 ⇓).
The proportion of women
with a «normal birth» (birth without induction of labour, epidural or spinal analgesia, general anaesthesia, forceps or ventouse
delivery,
caesarean section, or episiotomy9 10) varied from 58 % for planned obstetric unit births to 76 % in alongside midwifery units, 83 % in freestanding midwifery units, and 88 % for planned home births; the adjusted odds of having a «normal birth» were significantly higher in all three non-obstetric unit settings (table 5 ⇓).

Szabo wanted a vaginal
delivery, and argued
with hospital executives, but they stood firm: They refused to do vaginal births after
Caesareans (VBACs) because they have a slightly higher risk for complications.
Labor induction has been increasing since the early 1990s, 1 and the rate is running at about 20 % for pregnancies at term.2, 3 Induction of labor compared
with spontaneous labor is associated
with adverse maternal outcomes, including at least a doubling in the
caesarean delivery rate, 4,5 25 — 50 % increase in instrumental vaginal
delivery rate, 3,5 higher postpartum hemorrhage rate, 5 and prolonged labor.5 Neonates born after induced labor are more likely to have low Apgar score and low umbilical cord blood pH. 5

with normal
delivery.
A small study found that twice as many women (28 %) who were vitamin D deficient at the time of giving birth had a
Caesarean delivery compared
with those
with normal levels (14 %).
The authors of the study recognise the risks associated
with an elective
caesarean, and decisions about subsequent mode of
delivery in women who had a severe perineal tear in an earlier pregnancy must be weighed against the clinical and psychological impacts of severe perineal tearing.

delivery with a singleton breech presentation at term in a hospital in the USA today will have a
Caesarean delivery,» said Porto.
Only 29 - 36 percent of the births of women
with earlier
caesarean sections are vaginal
deliveries in these countries.
Other studies have shown that planned
caesarean deliveries are associated
with an increased risk of death or health complications in the newborn.
Exclusions: non-English-speaking women,
caesarean delivery, postpartum complications, infants
with congenital abnormalities or morbidity
For three analyses (Analysis 1.1 regional analgesia, Analysis 1.2
caesarean delivery and Analysis 1.16 episiotomy) there was a some suggestion of asymmetry, though in all cases this was due to two small trials
with large treatment effects in the same direction (Figure 3; Figure 4; Figure 5).

Compared to vaginal
deliveries,

with an increased risk of complications like placental abruption (when the placenta separates from the wall of the uterus before
delivery) and placenta previa (when the placenta partially or completely covers the cervix) in women who delivered their first child via
caesarean section.
