with the course content of the Baby Friendly Hospital Initiative were short - lived and not sustained at home unless implemented in combination
with post-natal home visits.35 Similarly in Italy, training of staff
with an adapted version of the Baby Friendly course content resulted in high
breastfeeding rates at discharge,
with a rapid decrease in the days after leaving hospital.36 In contrast, a cluster randomized trial in Belarus (PROBIT) found an association between an intervention modelled on the Baby Friendly Initiative
with an increased duration of
breastfeeding37 an association also reported from an observational study in Germany.38

Breastfeeding with HIV, is breast still best?
I don't believe the emotions felt by
mothers who don't
breastfeed or who wean early are as simple as «guilt»: when we really examine
mothers» feelings about things gone wrong, it is rarely guilt that they are expressing, especially about not
breastfeeding or not
breastfeeding as long or as completely as they would have liked to: well informed
mothers who reach
for the bottle after a struggle
with breastfeeding know they have done the best they could
with the resources they had at the time (health, energy, knowledge,
support)-- these
mothers may feel deeply sad and disappointed, they may be grieving, but guilt isn't an appropriate label
for these overwhelming feelings of loss
for themselves and their babies.
The announcement coincides
with World
Breastfeeding Week 2014, where the hospitals and other health partners — who are members of the Philadelphia Multi-Hospital
Breastfeeding Task Force **, will be declaring Philadelphia «The City of Motherly Love» and encouraging members of the public to sign a pledge showing their
support for nursing
mothers in the city.
So once again, the criticism about
breastfeeding really points to a lack of
support for new
mothers, rather than problems
with breastfeeding per se.
We did not identify any studies from those excluded from the publication «

This book is
for anyone who has ever struggled
with breastfeeding,
supported new

One compared volunteer counsellor
support in antenatal and postnatal period to usual care (Graffy 2004), one compared proactive
with reactive telephone
support for mothers living in disadvantaged circumstances (Hoddinott 2012), one compared
breastfeeding support and encouragement
for mothers of preterm infants to usual care (Junior 2007), one compared home nurse visits to usual care
for «well»
breastfeeding newborns and
mothers (Paul 2012), one compared prenatal paediatric home visits to usual care (Serwint 1996), and one compared
support from a close female confidante to usual care (Winterburn 2003).
As the babies get older, workplace
support and facilities may be especially important
for mothers of multiples in long - term maintenance of
breastfeeding (Gromada 1998), while further advice and
support may be needed during weaning to help prevent problems
with milk stasis and mastitis (Gromada 1998).
There is a need
for well - designed, adequately powered studies of interventions designed
for women
with twins or higher order multiples to find out what types of education and
support are effective in helping these
mothers to
breastfeed their babies.
